Abstract

¿¬±¸¸ñÀû: ¾Ç±¸°­°èÀÇ °Ç°­À» ½É¹ÌÀû, ±â´ÉÀûÀ¸·Î À¯Áö ¹× ÁõÁø½ÃÅ°´Â °ÍÀ» ¸ñÀûÀ¸·Î Ä¡¾ÆÀÇ ÇüÅ ÀÌ»óÀ̳ª °á¼Õ µîÀ» °®°í Àִ ȯÀڵ鿡°Ô °íÁ¤¼º º¸Ã¶ ¼ú½ÄÀÌ º¸ÆíÀûÀ¸·Î ½Ã¼úµÇ¾î ¿À°í ÀÖ´Ù. ȯÀÚ ½º½º·ÎÀÇ °ü¸® ºÎÁ·°ú ´õºÒ¾î ºÎÀûÀýÇÑ ±â°ø °úÁ¤À¸·Î ÀÎÇØ º¸Ã¶¹°°ú ¿¬°üµÈ ÇÕº´ÁõÀÌ ¹ß»ýÇÒ ¼ö ÀÖ´Ù. À̸¦ ÇØ°áÇÏ°íÀÚ °íÁ¤¼º º¸Ã¶¹°ÀÇ Àå±â°£ÀÇ ÀÓ»ó »óÅ Á¶»ç¿¡ ¸¹Àº ³ë·ÂÀ» ±â¿ï¿© ¿ÔÀ¸³ª ÀÚ·á ¼öÁý¿¡ ¾î·Á¿òÀÌ ¸¹¾Ò´Ù. ¿¬±¸Àç·á ¹× ¹æ¹ý: 2007³â 4¿ù¿¡¼­ 9¿ù±îÁö 6°³¿ù°£ ºÎ»ê´ëÇб³º´¿ø Ä¡°ú º¸Ã¶°ú¿¡ ³»¿øÇÑ ÃÊÁø ȯÀÚ Áß °íÁ¤¼º º¸Ã¶¹°À» ÀåÂøÇÏ¿© »ç¿ë ÁßÀΠȯÀÚ¸¦ ´ë»óÀ¸·Î º¸Ã¶¹°°ú °ü·ÃÇÑ Á¾ÇÕÀûÀÎ »óŸ¦ Á¶»çÇÏ¿´´Ù.

°á°ú ¹× °á·Ð: 1. Àüü °íÁ¤¼º º¸Ã¶¹°ÀÇ ÃßÁ¤ ¼ö¸íÀº 10.0³âÀ̾ú°í, Æò±Õ ÀåÂø ±â°£Àº 8.6 ¡¾ 0.6³âÀ̾ú´Ù. 2. ºÎÀ§º° ÃßÁ¤ ¼ö¸íÀº Â÷ÀÌ°¡ ¾ø¾úÀ¸³ª (P > .05), ¼º°ø·üÀº ÇϾǰú ±¸Ä¡ºÎ º¸Ã¶¹°¿¡¼­ ³ô¾Ò°í (P < .05), ÀüÄ¡-±¸Ä¡ È¥ÇÕÇü º¸Ã¶¹°¿¡¼­´Â ½ÇÆÐÀ²ÀÌ ³ô¾Ò´Ù (P < .05). 3. Àç·áº° ÃßÁ¤ ¼ö¸íÀº ±Ý¼Ó°üÀÌ 14.0³âÀ¸·Î °¡Àå ±æ¾ú°í, ±ÝÇձݰü (10.0³â), ±Í±Ý¼Ó µµÀç°ü (10.0³â), ºñ±Í±Ý¼Ó µµÀç°ü (8.0³â) ¼øÀ̾ú´Ù (P < .05). ±×·¯³ª ±Ý¼Ó°üÀº ½ÇÆÐÀ²ÀÌ ³ô¾Ò°í, ¼º°ø·üÀº ±ÝÇձݰü°ú ±Í±Ý¼Ó µµÀç°ü¿¡¼­ ³ô¾Ò´Ù. 4. Å©±âº° ÃßÁ¤ ¼ö¸íÀº Â÷À̸¦ º¸ÀÌÁö ¾Ê¾ÒÀ¸³ª (P > .05), ¼º°ø·üÀº ´ÜÀÏ°ü¿¡¼­ ³ô¾Ò°í (P < .05), ½ÇÆÐÀ²Àº 3-À¯´Ö À̻󿡼­ ³ô¾Ò´Ù
(P < .05). 5. ´ëÇÕÄ¡ Á¶°Çº° ÃßÁ¤ ¼ö¸íÀº Â÷ÀÌ°¡ ¾ø¾úÀ¸³ª (P > .05), °íÁ¤¼º ¹× °¡Ã¶¼º ±¹¼Ò ÀÇÄ¡°¡ ´ëÇÕÄ¡ÀÎ °æ¿ì ½ÇÆÐÀ²ÀÌ ³ô¾Ò°í, ÀÚ¿¬Ä¡ÀÏ °æ¿ì ¼º°ø·üÀÌ ³ô¾Ò´Ù (P < .05). 6. °íÁ¤¼º º¸Ã¶¹°ÀÇ ÇÕº´ÁõÀº Ä¡¾Æ ¿ì½ÄÁõ (23.0%), Ä¡ÁÖ Áúȯ (19.3%), Ä¡¼ö Áúȯ (16.9%) ¼øÀ̾ú°í, ±â°èÀû ¹®Á¦Á¡Àº º¯¿¬ °áÇÔ (28.2%), º¸Ã¶¹°ÆÄÀý (6.7%), À¯Áö·Â »ó½Ç (4.8%) ¼øÀ̾ú´Ù. º¸Ã¶¹°À» Á¦°ÅÇÑ ÈÄ ÀÜÁ¸ Ä¡ÁúÀº °íÁ¤¼º º¸Ã¶¹°ÀÇ ÇÕº´ÁõÀ¸·Î ÀÎÇÏ¿© 30.1%°¡ ¼öº¹ ºÒ°¡´ÉÇÑ »óÅ¿´´Ù.

Statement of Problem: Restoring and replacing teeth with fixed prostheses commonly used in dental practice. Because of improper oral hygiene care and inaccurate laboratory procedure, complications of fixed prostheses were found in the mouth of patients. Although many efforts have been continually made to obtain the data of long term prognosis of
fixed prostheses, it was difficult to do it.

Purpose: The purpose of this study was to evaluate the clinical status of fixed prostheses.

Material and Methods: In order to assess the clinical status of fixed prostheses, a total of 161 individuals (aged 17-85, 99 women and 62 men with 1596 unit of fixed prostheses, and 1169 abutments) who first visited the Department of Prosthodontics, Pusan National University Hospital, between April to September, in 2007 were examined.

Results and Conclusion: The results of this study were as follows: 1. Length of service of fixed prostheses was 8.6¡¾0.6 years (mean), 10.0 years (median). 2. Location of fixed prostheses was found to have no statistically significant
influence on longevity of fixed prostheses (P > .05). But, the success rate was high in posterior region and in mandible where the failure rate was high in combination (P < .05). 3. Longevity of fixed prostheses made of metal was longest (mean: 13.0 ¡¾9.3, median: 14.0), gold, precious ceramic, non-precious ceramic trailing behind (P < .05). 4. Number of
units in fixed prostheses was found to have no statistically significant influence on longevity of fixed prostheses (P > .05). But, the success rate was high in Single-unit and the fail-ure rate was high in over 3-unit (P < .05). 5. Condition of opposing dentition was found to have no statistically significant influence on longevity of fixed prostheses (P > .05). But,
the success rate was high in natural dentition (P < .05). 6. Defective margin (28.2%), dental caries (23.0%), periodontal disease (19.3%), periapical disease (16.9%) were frequent complications. In 30.1% of the cases, abutment state after removing fixed prostheses was needed to be extracted.
